Under Ontario’s Municipal Freedom of Information and Protection of Privacy Act (the Act), individuals have the right to request access to records held by public sector institutions, including access to their own personal information, where those records are under the institution’s custody or control.

Formal requests under the Act must be submitted using this online Access/Correction Request Form and are subject to a $5 application fee for each request.
